What we do

We are a team of health professionals with diverse skills including nutrition, nursing, social work, population health, psychology and teaching. We have teams based in Port Macquarie, Kempsey and Coffs Harbour.

We work in our local community across the Mid North Coast of New South Wales implementing evidence-based preventive health programs to improve population health. Our programs help individuals to:

  • maintain a healthy weight
  • keep active and healthy
  • reduce the risk of falls and injuries
  • help people to quit smoking
  • help people to reduce their alcohol consumption.

We deliver a range of programs across the lifespan in a variety of settings including early childhood education services, schools, aged care services and health clinics.

Our programs work towards reducing lifestyle related risk factors and can help to reduce chronic disease and reduce hospitalisations by keeping people healthier for longer.

Locally we work with local community members, schools, health professionals, local councils and other key partners to ensure that our programs maintain a place-based approach to health and wellbeing. We understand that where we live has an important role in shaping our health.

We all have a role to play to help build healthier communities and improve the health and wellbeing of every individual who live within our community.

We all have a role to play to help individuals make healthier lifestyle choices and lead healthy and active lives. There are a number of programs available that are designed to provide you with a referral pathway for your patients.

Health promotion is an evidence-based approach to improving the health of communities and individuals. As our community needs change we need to adapt programs to ensure they are achieving the desired outcomes.
