To refer to outpatient Physiotherapy Services at Kempsey District Hospital, email, fax or mail your referral to:

Referrals are triaged according to assessed urgency.

Referral information must include the following minimum details:

  • personal information: name, DOB, current contact number, address
  • reason for referral/history of presenting complaint including date of injury (or time frame of symptoms)
  • past medical history
  • medications.
