MNC Virtual Care

MNC Virtual Care delivers prompt access to quality virtual care to provide early assessment and intervention to patients within the Mid North Coast Local Heath District at risk of presentation to hospital or to support early discharge from hospital.

Our friendly healthcare team including GPs, nurses and allied health professionals are highly trained to assess and support you with:

  • acute respiratory illnesses (including COVID)
  • gastro and other viral illnesses
  • quick access to support for chronic care conditions (including COPD, asthma, heart failure and diabetes or action plans for these conditions)
  • medication support & prescriptions (scripts for controlled drugs not provided)
  • finding a GP or health service.

We can offer care at home via our virtual medical team if you can’t get a timely appointment with your GP.

Please note: If your symptoms are severe, you are seriously hurt or sick, please call 000 or present to your nearest Emergency Department for urgent treatment.

Our service does not provide support for:

  • patients requiring face to face examination, assessment or intervention at primary review
  • acute trauma / surgical
  • chest pain
  • abdominal pain
  • renal failure
  • pain management
  • palliative care – end stage illness
  • aged care/RACF
  • dental and oral health
  • mental health
  • drug and alcohol
  • pregnancy related concerns.

MNC Virtual Care is a collaborative, community-based multidisciplinary service, (medical, nursing, and allied health), maximising virtual care that provides early assessment and intervention to patients within the MNCLHD at risk of presentation to hospitals.

MNC Virtual Care also provides the option for early supported discharge from hospital where a Virtual Care modality can be utilised to provide care in the home.

Service Criteria – conditions that are eligible:
  • acute respiratory illness (including COVID)
  • asthma
  • chronic obstructive pulmonary disease (COPD)
  • mild pneumonia
  • cellulitis (excluding head region) – contact service for clinical eligibility and pathway
  • pulmonary embolism (inpatient/ED referrals only) – contact service for clinical eligibility and pathway
  • diabetes monitoring / insulin titration
  • gastroenteritis – acute diarrhoea and vomiting requiring symptomatic relief and fluid status monitoring.
  • medication support: patients presenting to emergency departments for medication or assistance with scripts, or those who have inadequate script supply post discharge medication education, N.B. S8 / opiates out of scope (in line with service type requirements Dept Health)
  • service linkage including access to primary care providers, care services, allied health
  • multi-disciplinary team (medical, nursing and allied health) input, review and referral pathways
  • late-stage chronic disease care navigation through Enhanced Community Care service

MNC Virtual Care provides a pathway for patients who are unable to access their GP/ primary care provider for the referred areas of concern

You can refer your patient to MNC Virtual Care for assessment if they meet the following criteria:

  • patients who are either residents of MNCLHD or are temporarily residing within the area
  • meet the key areas outlined above.
Current exclusions and referrals will not be accepted for:
  • patients requiring face to face examination, assessment or intervention at primary review
  • acute trauma / surgical
  • chest pain
  • abdominal pain
  • acute renal failure
  • pain management
  • palliative care – end stage illness (refer to palliative care services)
  • aged care/RACF/correctional facility
  • dental and oral health – can accept if self-referred, to assist with care navigation – ED presentations to be referred along dental pathways.
  • mental health
  • drug and alcohol
  • hyperemesis gravidarum.
