Heart Failure Program

The Heart Failure Program provides holistic care and support to patients and their families who have recently been diagnosed with heart failure.

The program mostly focusses on strategies to best manage your condition when you leave hospital in your home environment.

A comprehensive plan is devised in consultation with your general practitioner (GP) and cardiologist to help you recover when you leave hospital and referral to other services if required.

The core group of people eligible for cardiac rehabilitation are those who have had:

  • myocardial infarct (heart attack)
  • re-vascularisation procedures
  • stable or unstable angina
  • controlled heart failure
  • other vascular or heart diseases.

People with other presenting problems, such as Type 2 diabetes or multiple risk factors may also participate in cardiac rehabilitation.
