Respiratory Liaison Program
This program includes ward visits, home visits and phone calls to provide short term post discharge emotional support and education on respiratory disease and self-management.
Service exclusions apply for this service.
Respiratory liaison is for people with many forms of the following pulmonary conditions:
- COPD
- Asthma
- Bronchiectasis
- Idiopathic Pulmonary Fibrosis
A referral from your GP is preferable but you can refer yourself.
