Chronic Care for Aboriginal People (CCAP)

CCAP is a state wide program aimed at improving the health of Aboriginal and/or Torres Strait Islander People over the age of 15 years, who are living with a chronic disease such as:

  • diabetes
  • kidney disease
  • heart disease
  • respiratory disease.

The Aboriginal Chronic Care Worker (ACCW) can support patients in the community to:

  • manage the chronic disease at home
  • understand the chronic disease
  • maintain current care plans or implement new appropriate care plans
  • access specialist health professionals
  • access specialist community health services 
  • link to rehabilitation, education and other programs
  • link to further supports and services in the community
  • provide advocacy
  • manage appointments.
