Keeping Aged Care Residents at Home

The Aged Care Outreach Service brings urgent specialist care directly to aged care homes through virtual consultations and, when needed, a rapid-response ‘Flying Squad’ nursing team who attend on-site. Since August 2025, 351 referrals have been managed with an 89% emergency department avoidance rate keeping residents safe, comfortable and connected to family in the place they call home.
