The Rehabilitation /Stroke Service provides gold standard evidence based care by a dedicated multidisciplinary team to patients experiencing acute stroke and if required the patient is able to flow through to the Rehabilitation Unit to maximise functional improvement ready for discharge.

Referral required for admission to the Rehab Unit, Stroke Unit admission via Emergency Department.

The Rehabilitation  Unit is an 8 bed unit, serving Wauchope and Port Macquarie  communities providing an in-patient rehabilitation services.

The in-patient rehabilitation  unit is  run by a  team of highly dedicated health professionals including consultant rehabilitation physician, nurses specializing in rehabilitation,  physiotherapists, occupational therapist, speech pathologist, dietician, healing touch therapist, and social worker.

Rehabilitation is specialized healthcare dedicated to improving, maintaining or restoring physical strength, mobility and cognition.   Rehabilitation programs help people recover from illness or surgery with the support of a Multidisciplinary Team (MDT). 

Programs help people regain their strength and / or mobility, re-learn skills and find new ways of doing things. Program aims to achieve the highest level of independent functioning for the individual.  

Rehabilitation MDT includes Rehabilitation Physician, Rehabilitation Nurses, Physiotherapist, Occupational Therapist, Speech Therapist, Social Worker, Dietician and Allied Health Assistant.  The MDT health professionals understand that each person’s needs and their therapy objectives are different. 

The rehabilitation team works with patients to design a personalized therapy program with individual goals and objectives.  You are an active member of the rehabilitation team and your level of motivation and participation are vital to achieving your program goals.

Rehabilitation Program focus is on assessing level of impairment, building strength, improving mobility and fitness, assessing and helping with daily activities, managing pain, and discharge planning that enables a safe return to their home environment / residential care facility.

To refer to the service contact  02 6589 8000 , ask for Rehabilitation Unit and request  a referral form to  be sent.  Form must be completed by MDT  and medical officer and returned  by facsimile.  

A  follow-up phone call is required  to  our  admitting  Rehabilitation Consulting Physician requesting transfer of care for rehabilitation health care services at Wauchope Hospital Rehabilitation Unit.
