Palliative Care

The palliative care service is provided for people of all ages with a life limiting illness who have little or no prospect of a cure. People with a life limiting illness can include those with a diagnosis of a cancer or end stage non malignant disease.

The aim of the palliative care service is to provide timely and appropriate care and support to clients living with a life limiting illness and their families. The palliative care service works collaboratively in the management and care provision with other service providers.

The service may visit clients at home, in hospital or residential aged care facilities. The care provided will vary according to the needs of the client and may include:

  • the assessment and management of symptoms related to their life limiting
    illness
  • a consultancy service by the palliative care specialist and/or nursing staff
  • nursing care
  • support from the palliative care social worker
  • shared care with other service providers 
  • loan of equipment to assist in care provision at home
  • bereavement support.

The patient must be aware of the referral.

Referrals will be accepted from:

  • hospitals
  • Mid North Coast Cancer Institute
  • outpatient clinics
  • community nurses
  • general practitioners
  • other health professionals
  • self referral.
