Aims to assist carers to continue caring for people with dementia and challenging behaviours at home by providing respite, practical support and education, thus maintaining the independence health and wellbeing of both and preventing inappropriate premature admission of people with challenging behaviours to residential facilities.
Criteria for Referral to the Service
Carers of people who present with dementia and challenging behaviour and who live in rural/remote areas. This service is aimed at targeting carers who have difficulty accessing other services or existing support services are limited and insufficient. This service includes carers who reside in the rural and remote areas of the Hastings and Macleay local government areas.
Referral may be by phone during normal office hours or fax at any time. Intake for referrals are taken by the intake staff of the Aged Care Assessment Team. The assessment will be attended by the staff of the Mobile Respite Service on a planned, pre-booked basis. Priority of need will be determined through assessment and consultation with the carer by the Mobile Respite Service staff.
