Respiratory Liaison Program
This program includes ward visits, home visits and phone calls to provide short term post discharge emotional support and education on respiratory disease and self-management.

- Must have a primary chronic respiratory diagnosis.
- Be willing to receive support.
- Reside in Coffs Harbour LGA.
Referrals from Coffs Harbour Health Campus only, health professionals.
