Cardiac Rehabilitation Program

The Cardiac Rehabilitation Program offers education and exercise for patients who have cardiovascular disease or have had a cardiac event. Significant benefit can be gained through cardiac rehabilitation by patient interventions such as self-management strategies, medication, education, physical activities and more.

The goals of cardiac rehabilitation include establishing an individualised plan of care to help the participant regain physical strength, make healthy lifestyle choices, reduce their risk of further cardiac problems, and improve general health and quality of life.

Cardiac rehabilitation is an option for people with many forms of heart disease. In particular, those with diagnosis and / or medical history:

  • heart attack
  • coronary artery disease
  • heart failure
  • peripheral artery disease
  • chest pain (angina)
  • cardiomyopathy
  • certain congenital heart disease
  • coronary artery bypass surgery
  • angioplasty and stents
  • heart or lung transplant
  • heart valve repair or replacement
  • pulmonary hypertension.

People with other presenting problems, such as type 2 diabetes or multiple risk factors may also participate in cardiac rehabilitation.

Service exclusions apply for this service.
